How to Fill out a Witness Slip

Filling out a witness slip is a quick and easy way to let legislators know whether you support or oppose a bill.

Witness slips are read during a committee hearing and help inform legislators as to where people stand, allowing informed decisions when voting.

Here are some tips for completing a witness slip:

  • In Section I, fill out your information. In the 'Firm/Business or Agency' and 'Title' boxes, enter "Self"

  • In Section II Representation, enter “Self” into the box

  • In Section III Position, select your position on the bill: "Proponent" if you support it and "Opponent" if you are against it.

  • In Section IV Testimony, mark "Record of Appearance Only

  • Click ‘Creat (Slip)’ to submit your witness slip.
